Healthcare Improvement Scotland

From 1st April, 2022, there will be changes to the pre-employment occupational health clearance process. The SBU director, as part of their approval of the post, will be responsible for checking that a Workplace Hazard Identification Form has been completed for all vacancies.  Guidance notes for managers with supporting explanations of the hazards, is available.

This form should be completed by appointing manager and sent to Recruitment with the vacancy information for specified post. Occupational Health will require this information on receipt of a completed pre-employment health questionnaire.

This information will allow occupational health to assess the applicant’s health status against identifiable workplace hazards. Occupational Health clearance for the job post is based on the information provided on this form.

Following receipt of the form, the new start may be contacted by the OHS to provide further information.

 

National Services Scotland
