A Notification of Change (NoC) is a request made by an employee which enables a component of their working arrangement to be changed subject to approval and most of these changes will be processed through the Forms Handling Portal or an alternative form as list below.

For the best experience and to avoid processing delays, we recommend that employees complete their NoC request on the Forms Handling Portal. Each request will then be automatically passed on to the relevant authoriser, starting with the employee’s line manager. Until the NOC portal is fully launched for all changes, some requests may still require a individual paper form. For the complete list of which method to request your change please see below. 

Please note: where there has been a line manager change, it will take 2 working days for the Forms Handling Portal to be updated with this information.

This Notification of Change Form includes a Rota Review Group section, which must be completed.  Please note, this form should not be used to change hours, the line manager (or proxy), should amend a staff members hours on eESS using the Standard Operating Procedure in the Knowledge Lab on the intranet.

 

NHS Education for Scotland

A Notification of Change (NoC) is a request made by an employee which enables a component of their working arrangement to be changed subject to approval. It should be noted that all NoC forms must be signed off by the relevant Managers/Directors before the HR team can process the change.

Public Health Scotland

A Notification of Change (NoC) is a request made by an employee which enables a component of their working arrangement to be changed subject to approval and most of these changes will be processed through the Forms Handling Portal or an alternative form as list below.

From 1st April 2020 Public Health Scotland will bring together staff previously employed by NSS and NHS Health Scotland alongside new PHS employees, meaning different routes for staff to access HR information.  Please refer to the option below that best describes your circumstances to ensure you refer to the correct information. 

• I have TUPE transferred from NHS Health Scotland into Public Health Scotland – Please raise your query with HR via Contact Us
• I have TUPE transferred from NSS to Public Health Scotland – please select the National Services Scotland tab above to access all information.
• I am an employee in Public Health Scotland – please select the National Service Scotland tab to access all information.

 

This form below should be used (until portal access is available) for changes in your terms and conditions of employment.  Please submit the fully completed and signed document to your line manager, who will forward the request to the proxy users for the HR Portal.  Your line manager will receive email confirmation once the transaction has been processed.  Please be aware that any forms not received by HR by the 8th of the month (payroll cut-off) may not be reflected in that month’s salary.

 

*Please note that this manual form is only applicable to former Health Scotland employees, as former NSS employees should have portal and eESS access.

Scottish Ambulance Service

The State Hospital

A Notification of Change (NoC) is a request made by an employee which enables a component of their working arrangement to be changed subject to approval. It should be noted that all NoC forms must be completed with the relevant financial codes (see below in 'For Manager' guidance) and signed off by the relevant Managers/Directors before the HR team can process the change.

Changes to personal details should be emailed to TSH.HREnquiries@nhs.net. If you are changing your name, please attach proof of your new name e.g. passport, marriage certificate, deed poll.

It should be noted that you must inform SPPA by yourself if you have any changes to personal details.
